To ensure compliance with OSHA Process Safety Management guidelines, Lyondell, Equistar and Millennium maintain a database of our service providers’ safety and health program information. The database is updated annually to ensure we have the most current information possible. The electronic forms available on this web site are designed to expedite the U.S. service provider approval process.

Provider Approval Process

1. Access the supplier pre-qualification information page using the link to the left.
2. Open the Standardized Pre-Qualification Form. Save the document to a local drive and complete the form.
3. Review the Lyondell, Equistar and Millennium Rules for Contractors. Complete the associated Contractor Acceptance Form at the back of the document. Create a scanned copy of the completed document (PDF is the preferred format, however we will also accept JPG or TIF documents.) Print the remainder of the document for your files.
4. Review our English Proficiency Letter.
5. Submit your completed Standardized Prequalification Form and the scanned copy of your Contractor Acceptance Form to PQF@lyondellbasell.com. In some cases, we may also request copies of your company Certificate of Insurance(1), Substance Abuse Policy and Safety, Health and Environmental Manual. Contractors who work in Louisiana must also provide an electronic copy of either their Louisiana Contractor’s License or proof of exemption. 

Subcontractors

Approved providers are responsible for ensuring that their subcontractors have been endorsed for use by Lyondell, Equistar and Millennium. Once the endorsement is finalized, it becomes the responsibility of the approved provider to ensure their subcontractors adhere to these procedures and complete the documentation process outlined above before they begin work at a Lyondell, Equistar or Millennium site.
